When you book a consultation, you will be asked to complete intake forms through Practice Better (my practice management platform). These forms collect the personal and health information outlined in Section 2.

Information submitted via website contact forms is used solely to respond to your enquiry and provide requested information.

By submitting a booking form or intake form, you consent to:

Storage of your records within Practice Better’s secure platform

I retain client health records for a minimum of 7 years from the date of last consultation, or 7 years after a child turns 18, in accordance with applicable Australian health records legislation.
